The Ozark Family Centered Medical Home will temporarily close beginning Dec. 26 due to provider shortages.
Those beneficiaries enrolled at Ozark will be moved to Leonard Wood Army Medical Home at General Leonard Wood Army Community Hospital.
All previously scheduled appointments will be honored by the same provider, on the same date and time at LWAMH. Full scope ancillary services are available within the hospital.
Patients will remain assigned to their current Primary Care Manager. If you have questions or would like to change your PCM please contact our Guest and Healthcare Relations team at 573.596.0418/7414.
