The training at the Mayo Multidisciplinary Simulation Training Center in Rochester, Minn., is the first practical exercise in the Army Reserve to enforce the teaching of the Team Strategies and Tools to Enhance Performance and Patient Safety model. Developed by the Defense Department's Patient Safety Program, is meant to mitigate what the Joint Commission, an independent organization that accredits medical facilities, found to be the leading root cause of adverse events in hospitals for the past decade; communication failures.
