ST. LOUIS - Sailors from Navy Reserve Military Sealift Command Far East Headquarters (NR MSCFE HQ) conducted Tactical Combat Casualty Care (TCCC) training as part of their four-day all-hands drill weekend at Navy Reserve Center (NRC) St. Louis, Nov. 14, 2024.

The TCCC course teaches Sailors life-saving techniques and strategies to treat combat casualties and reduce casualties onboard the ship and on the battlefield. The interactive training provides Sailors hands-on, realistic scenarios in administering first aid to casualties in the event of an active shooter or battlefield skirmish.

The more than 30 Sailors in attendance simulated providing care to other classmates and mannequins. The scenarios are a way to get as close as possible to a real-life emergency and live-fire situation using the knowledge learned and available equipment.

“I liked the interaction in the lessons of basic first aid, learning how we should check for wounds and properly put on tourniquets,” said Operations Specialist 1st Class Juan Amadiz, assigned to NR MSCFE HQ. “We learned the importance of why basic first aid is critical in saving lives.”

The attendees moved through different zones depending on the extent of the patient’s wounds while simulating different intensities of environmental stressors.

“I liked how we ran through the scenarios testing our skills, I think that was the fun part,” said Amadiz.

TCCC training consists of four tiers. Tier 1- All Service Member (ASM) training is designed to provide basic, evidence-based lifesaving skills for non-medical personnel at their duty stations or in preparation for non-combat deployments. Tier 2- Combat Lifesaver (CLS) training is designated for non-medical personnel tasked to provide additional medical support within their units during sustained combat or contingency operations. Tier 3- Combat Medic or Corpsman training is intended for medical personnel, to include some licensed medical practitioners. Tier 4- Combat Paramedic or Provider (Advanced Resuscitative) training is designated for advanced medical personnel, some licensed medical practitioners and select non-medical personnel that may need to provide prolonged advanced field care for injured Service Members.

“This is my second time taking this course, I took it two years ago at my local NRC in Columbus [Ohio] and this time I feel like the course has upgraded,” said Logistics Specialist 3rd Class Diane Ngo, assigned to NR MSCFE HQ. “The first time I took it we learned about M-A-R-C-H, which is massive hemorrhage, airway, respiratory, circulation, hypothermia/head injury and this time we learned more about the under-fire care, the tactical field care, and tactical evaluation care.”

The course equips individuals with essential skills to respond quickly and effectively in treating combat related trauma.

“I liked how they ran the training and how they got us involved because it is the best way to gain more knowledge and experience so that we’re able to use what we learned if we ever face an actual situation like this,” said Ngo. “After completing the course, I feel like I’m more prepared for situations like this even though many of us here aren’t in the medical field.”

NR MSCFE HQ augments MSCFE HQ Staff, providing support to expeditionary logistics, strategic sealift, expeditionary port operations and position/replenishment of combat cargo. NR MSCFE HQ also provides liaison officers to Fleet and Combatant Commanders for surge requirements in support of crisis response and contingency operations across U.S. Indio-Pacific Command area of responsibility.
